This plan is only honored at the Northcutt Dental office where your enrollment is registered and cannot be used at any other dental office.

Benefit Premiums
$199 Annually for the first family member
$799 Annually for a Family Membership
Family plans cover family members and children up to 18 years of age. Full-Time college students will be covered until age 23.
Benefits start immediately (no waiting period!) and you must remain on the policy for a minimum of one year. You will not receive a membership card, as your plan’s effective date will be on file with the front office.
